About the event

Presenter: Nicole Matthews, Director of the Social Care Network, Greater Health Now


Greater Health Now serves as an Accountable Community of Health (ACH) supporting south-central and eastern Washington. ACHs partner with regional organizations and state agencies to address health-related social needs, improve care coordination, and expand equitable access to essential services and supports.


Community Care Hubs are community-centered systems designed to streamline referrals and connect individuals to critical services and benefits. These hubs coordinate efforts across healthcare providers, behavioral health organizations, and community-based partners to ensure people receive culturally responsive, person-centered support. Community Care Hubs also help strengthen and grow the community-based workforce that provides care coordination and navigation services.


During this session, participants will:

  • Learn what Accountable Communities of Health are and how they operate across Washington State

  • Gain an understanding of the Community Care Hub model and how it supports coordinated care
